In order to process your membership application, you are required to submit proof of completion for an ACGME accredited program.
a) Email, scan, fax, or mail a copy of your ABPN board certificate in child psychiatry.
b) Submit a completed Training Verification
form for your child psychiatry program.
You must also submit two completed Professional Activity Verification forms
. This reference needs to be completed by an AACAP member to support your current professional duties.
Forms can be submitted by one of the following ways:
a) Electronic – download and email the enrollment verification form to the appropriate faculty; information is updated directly into the form; and faculty will return the completed enrollment form by email.
b) Scan – print out the enrollment verification form; scan the completed form and email to email@example.com
c) Fax – print the form and fax the completed form to AACAP Member Services at 202.464.0131.
d) Mail – mail the completed form to AACAP 3615 Wisconsin Ave., NW, Washington, DC 20016.
