In order to process your membership application, you must submit a completed Training Verification Form
Forms can be submitted by one of the following ways:
a) Electronic – download and email the enrollment verification form to the appropriate faculty; information is updated directly into the form; and faculty will return the completed enrollment form by email.
b) Scan – print out the enrollment verification form; scan the completed form and email to firstname.lastname@example.org
c) Fax – print the form and fax the completed form to AACAP Member Services at 202.464.0131.
d) Mail – mail the completed form to AACAP 3615 Wisconsin Ave., NW, Washington, DC 20016.
e) E-mail - ask your Program Director to email email@example.com
with your name, start/end dates, and a statement about the progress of your training.
